Patient Engagement for Health Webinar

This is a CAH Quality Director Call

This presentation, focused specifically for CAHs, will provide participants with information, tools, and resources on effectively engaging patients and family members to take a more active role in their own health care to prevent illness and more effectively manage their own care.

Objectives include:

• Understanding components of patient activation & empowerment
• Identifying strategies for encouraging patient self-management
• Reviewing tools and resources designed to improve patient health outcomes
