Is your organization ready for the new Colorado MOST?

Over the past year, the Colorado Advance Directives Consortium ("CADC") has made significant updates and changes to the Colorado Medical Orders for Scope of Treatment form, in line with feedback from the field and emerging national standards.The updated MOST form will "go live" on National Healthcare Decisions Day, April 16, 2015. Providers, first responders, and healthcare facility staff across Colorado will need training and updated information on the form and refinements to the utilization process.

Attend this important session on May 4 at the Donor Alliance in Lowry (East Denver) to learn about changes to the MOST and obtain training skills and materials so you can teach others in your organization and community about the new MOST. Two options for the training are available: attend the first 90 minutes online, from your location at no charge, to hear the essential updates; or attend in person, for only $25, for
  • a comprehensive review of the updated form and process,
  • tutorial in a proven best practice for the MOST “conversation,” and
  • training as a MOST educator for your community or organization.

In-person attendees will also receive an extensive toolkit of MOST resources, access to online slide decks and presentation materials, ongoing support from the CADC team, and more!

The in-person training will also qualify attending Medicaid providers to bill Medicaid for MOST counseling of Medicaid beneficiaries (eligible providers include physicians, advanced practice nurses, physician assistants, and licensed mental health providers).

Instructors for this session are the Colorado Advance Directives Consortium co-chairs: David Koets, MD, Chief Medical Officer of The Denver Hospice, and Jennifer Ballentine, MA, Vice President, Hospice Analytics. Other CADC members will attend for additional tutoring and insight.

To keep our costs low, we will provide beverages and a very short lunch break (so as to make the "most" of everyone's time). Please plan to bring your own lunch to the session.

May 4 Agenda:

11:00–12:30: More about the MOST: Summary of updates to the MOST form and process

12:30–1:00: Lunch break - please bring your own lunch!

1:00–2:00: The MOST Conversation: Best practices for how to complete a MOST that truly reflects patient wishes.

2:00–3:00: Train the Trainers: Skills, tools, and materials for conducting training in your organization or community
