Patient Centered Medical Home Designation

The Colorado Rural Health Center team assists clinics in pursuing designation by the National Committee on Quality Assurance ( NCQA)as Patient Centered Medical Homes ( PCMH). This work involves facilitating a team based approach within the clinic in complying with a set of standards to organize care around the patients needs and to coordinate and track care over time.The PCMH is a healthcare setting that facilitates partnerships between the patient, provider, and family.
