Healthy Transitions Colorado – Join the Campaign!

Healthy Transitions Colorado is a collaborative effort, focused on aligning and accelerating existing efforts to improve transitions of care for Coloradans. Join the Healthy Transitions Colorado campaign and be part of a broad and growing base of health care providers and partners who have pledged their commitment to help reduce avoidable hospital readmissions in Colorado.
