DNH X: Patient Safety Award Nominations Now Being Accepted

The Colorado Patient Safety Coalition is accepting nominations for patient safety awards to be presented at this year’s Do No Harm X Conference. Awards will be given according to the following criteria: Individuals, groups or organizations whose patient safety projects have demonstrated leadership and innovation in improving patient safety within Colorado through:

• Promotion of cultural change that encourages continuous improvement in patient safety,
• Leadership in the implementation of proactive strategies to improve patient safety, and
• Application of evidence-based improvement methods with empirical measurement of the results.

Awards will be presented during the conference on November 19th, 2010. The application deadline is November 1st, 2010. Award recipients must attend the conference to receive award. To make your nomination, please fill out this form and email to admin@coloradopatientsafety.org.
