Are you interested in a medical home model for children?

The Colorado Children’s Healthcare Access Program (CCHAP) is a non-profit organization created 5 years ago to ensure that every child enrolled in Medicaid and Child Health Plan Plus (CHP+) receives comprehensive health care from a primary care provider – a “medical home”. CCHAP is funded by several Colorado foundations to increase access for the under-served children of Colorado. CCHAP recruits, trains and supports pediatric and family practices who care for these children and helps the practice become certified with the state as a Medical Homes for Children on Medicaid. CCHAP also assists these practices by providing support services to help break down the historical barriers to caring for this population. CCHAP also links practices to community services for families, provided by community-based organizations, for example, mental health counseling, social services, transportation, care coordination, case management, and quality improvement coaching.

While certified RHCs do not currently receive extra funding for this program we’d love to get you involved. For more information on how you can participate please contact Anita Rich, Community Outreach and Quality Improvement for CCHAP at anita.rich@childrenscolorado.org or 720.777.5495
