New Combined CMS Notice Forms for Medicare certified HHAs, SNFs, Hospices, and CORFs

CMS has issued new notices that combine the Original Medicare (FFS) and Medicare Advantage notice into one form to be used effective May 1, 2012 for home health agencies, skilled nursing facilities—hospital swing bed providers, hospices, and comprehensive outpatient rehabilitation facilities. The new combined Notice of Medicare Non-Coverage CMS Form- 10123 is used to aler beneficiaries them that Medicare covered items and/or services are ending and to give beneficiaries the opportunity to request an expedited determination.
