ATTENTION: Providers Submitting Additional Documentation to Novitas
Effective May 1st, 2013, Novitas Solutions Inc. will return all documentation received that is not submitted with the below listed approved Novitas forms, which must be included with all faxed/mailed documentation submitted to our office. This includes all information submitted when Novitas requests additional documentation via the Additional Documentation Response (ADR) letter; or Medical Records requested during a Post Pay Review; or with the submission of PWK Part A/B documentation.
