Healthy Transitions Colorado

Since the launch of Healthy Transitions Colorado in early July, 63 organizations from across Colorado have signed the pledge to participate as a community partner in the statewide care transitions campaign aimed at reducing hospital readmissions and saving millions in health care dollars. The campaign's first live educational Webinar, "Implementing an Enhanced Home Health Program to Prevent Hospital Readmissions", will take place on September 18th from 12:30-1:30pm MT.
